 <title>Self-Esteem, Confidence and Relationship Satisfaction of Men With Erectile Dysfunction Treated With Sildenafil Citrate</title>

&lt;p&gt;&lt;b&gt;PURPOSE&lt;/b&gt;: We assessed the change in confidence, relationships and self-esteem, and its correlation with erectile function in men with ED treated with sildenafil citrate in the first United States based, double-blind, placebo controlled, randomized trial assessed by the validated SEAR. &lt;b&gt;MATERIALS AND METHODS&lt;/b&gt;: This 12-week flexible dose (25, 50 or 100 mg) trial determined change scores from baseline to end of treatment for the 5 SEAR components (Sexual Relationship domain, Confidence domain, Self-Esteem subscale [prespecified as the primary end point], Overall Relationship subscale and Overall score), and their correlations with the IIEF and event log data, as well as correlations between SEAR components and a general efficacy question at the end of treatment. &lt;b&gt;RESULTS&lt;/b&gt;: Compared with the placebo group (125 patients, mean age +/- SD 55 +/- 13 years, mean years ED 3.8 +/- 4.2), the sildenafil group (128 patients, mean age +/- SD 56 +/- 12, mean years ED 4.6 +/- 4.3) had significantly greater improvements in all 5 SEAR components (p less than 0.0001) and all sexual function measures. SEAR component scores showed significant correlations with IIEF Erectile Function domain scores (r range 0.34 to 0.69, p less than 0.0001), other IIEF domain scores (p less than 0.0001), percentage of successful intercourse attempts (p less than 0.0001) and frequency of erection that allowed satisfactory intercourse (p less than 0.0001). &lt;br&gt;&lt;/p&gt;
&lt;p&gt;&lt;b&gt;CONCLUSIONS&lt;/b&gt;:&lt;br /&gt;
&lt;i&gt;In this study of men with ED, sildenafil produced substantial improvements in self-esteem, confidence and relationship satisfaction&lt;/i&gt; as measured by SEAR scores, which showed moderate to high positive correlations with IIEF scores.&lt;/font&gt;&lt;/p&gt;

&lt;b&gt;Atherosclerosis and Hypertension (high blood pressure)&lt;/b&gt;  &lt;br&gt;

Two very common conditions.  The narrowing and hardening of arteries (atherosclerosis) reduces blood flow to the reproductive organs.  When coronary arteries (arteries that supply blood to the heart muscle) are narrowed by atherosclerosis, heart attacks and angina occur. When cerebral arteries (arteries that supply blood to the brain) are narrowed by atherosclerosis, strokes occur. Similarly, when arteries to the penis and the pelvic organs are narrowed by atherosclerosis, insufficient blood is delivered to the penis to achieve an erection.  It is not clearly known how essential hypertension causes erectile dysfunction, however, it is thought to decrease the levels of nitric oxide produced by the arteries in the penis.  
&lt;br&gt;&lt;br&gt;
&lt;b&gt;Low Testosterone Levels&lt;/b&gt;&lt;br&gt;
Testosterone is necessary for sex drive (libido), sperm production, and maintaining nitric oxide levels in the penis.  Therefore men with hypogonadism (poor function of the testes which results in lowered testosterone production) may display low sex drive, poor sperm production, and erectile dysfunction.  This is a routine test that can be ordered by your doctor.
&lt;br&gt;&lt;br&gt;
&lt;b&gt;Mental-Emotional Conditions&lt;/b&gt;&lt;br&gt;
Psychological factors may be responsible for erectile dysfunction.  Some common afflictions include anxiety, depression, guilt, low self-esteem, and performance fears.
Negative feelings that you express toward your sexual partner — or that are expressed by your sexual partner — such as resentment, hostility or lack of interest also can be a factor in erectile dysfunction.
&lt;br&gt;&lt;br&gt;
&lt;b&gt;Habitual Substance Use&lt;/b&gt;&lt;br&gt;
Alcohol can cause nerve damage and atrophy (shrink) the testicles leading to lowered testosterone production.  Marijuana, heroin, cocaine, ecstasy, methamphetamines, and other recreational substances can also contribute to erectile dysfunction.

&lt;b&gt;Medications&lt;/b&gt;&lt;br&gt;
Erectile dysfunction can be caused by many common medications.  Some of these include drugs for hypertension, allergies and congestion, depression and anxiety, as well as some tranquilizers, and appetite suppressants.
